Food Sensitivities: What They Are, How They Differ, and What You Can Do About Them 

Many people notice that certain foods do not suit them well. They may feel bloated, develop skin symptoms, experience headaches, fatigue, digestive discomfort, or simply feel “inflamed” after eating particular foods. These reactions are often described broadly as food sensitivities, but the term actually covers several very different mechanisms. 

Understanding the difference matters, because not every reaction to food is an allergy, and not every digestive symptom is caused by the immune system. 

What are food sensitivities? 

The phrase food sensitivity is often used as a general umbrella term for unwanted reactions to foods. In practice, these reactions may fall into three broad categories: 

  1. Food allergy, usually mediated by IgE antibodies 
  2. Food intolerance, which is usually not immune-mediated 
  3. Non-classic or delayed immune reactions, sometimes discussed in relation to IgG or IgA, although these are more complex and not always straightforward to interpret clinically 

That is why two people can react to the same food in completely different ways. One may have an immediate allergic reaction, while another may simply have bloating or discomfort because they cannot digest part of that food properly. 

The main types: IgE, IgG and IgA 

  1. IgE-mediated reactions: classic food allergy 

IgE-mediated food allergy is the best-defined and most clinically established type of immune reaction to food. In this situation, the immune system identifies a food protein as a threat and produces IgE antibodies against it. These reactions often occur quickly, sometimes within minutes to a few hours after eating. 

Typical symptoms may include: 

  • hives or itching 
  • swelling of the lips, tongue, or throat 
  • wheezing or breathing difficulty 
  • vomiting
  • abdominal pain 
  • dizziness 
  • in severe cases, anaphylaxis 

This is the type of reaction that requires proper allergy evaluation and, in some cases, emergency treatment such as epinephrine. 

  1. IgG reactions: delayed and controversial territory 

IgG antibodies are sometimes measured in commercial “food sensitivity” panels. However, major allergy organizations state that IgG testing is not a validated way to diagnose food allergy or food intolerance. In many cases, IgG may simply reflect that a person has been 

exposed to a food and developed normal immune recognition or tolerance to it, rather than proving that the food is harmful. 

That said, in real-life clinical practice, some people do report symptoms that appear delayed, less dramatic, and harder to track than classic allergy symptoms. These may include bloating, fatigue, headaches, skin flares, nasal congestion, or digestive discomfort after repeated intake of certain foods. The challenge is that these symptoms are often multifactorial, so interpretation requires careful clinical context rather than relying on a lab number alone. 

A practical way to think about it: 

  • IgE reactions are usually immediate and potentially dangerous. 
  • IgG results may sometimes be discussed as part of a broader clinical picture, but they are not considered definitive proof of a harmful food reaction on their own. 
  1. IgA reactions: mucosal immune response 

IgA is an antibody that plays an important role in the mucosal immune system, especially in the gut. Food-specific IgA is sometimes included in specialty testing, with the idea that it may reflect immune interaction at the intestinal surface. However, like IgG, food-specific IgA testing is not widely accepted as a standalone diagnostic tool for food sensitivity. It may offer context in selected cases, but it should not replace a careful medical history, symptom review, and structured dietary assessment. This is an inference based on the broader allergy guidance that antibody tests beyond appropriate IgE-based evaluation do not by themselves establish diagnosis. 

In other words, IgA can be biologically interesting, especially when thinking about gut barrier health and mucosal immunity, but it is not the same as proving a clinically meaningful food allergy. 

So what is the difference between IgE, IgG and IgA? The simplest summary is this: 

  • IgE = classic allergy, often rapid, sometimes severe, and medically well established.
  • IgG = exposure-related antibody that is often overinterpreted; not recommended alone for diagnosing food sensitivity. 
  • IgA = part of mucosal immunity, potentially informative in context, but not a definitive standalone diagnostic marker for food reactions. 

What is lactose intolerance? 

Lactose intolerance is very different from an immune reaction. It is not an allergy and does not involve IgE, IgG, or IgA antibodies in the usual sense. It happens when the small intestine does not produce enough lactase, the enzyme needed to digest lactose, the natural sugar found in milk and dairy products. Undigested lactose passes into the colon, where bacteria ferment it, leading to gas, bloating, abdominal pain, and diarrhea. 

Common symptoms of lactose intolerance include: 

  • bloating 
  • gas 
  • abdominal cramps 
  • loose stools or diarrhea 
  • nausea after dairy intake

  • Milk protein IgG positivity = may reflect exposure, not necessarily disease; it must be interpreted carefully and not automatically treated as proof that dairy is causing symptoms. 

A person with lactose intolerance may tolerate lactose-free dairy but still react to milk proteins if they have a true milk allergy. On the other hand, a person may avoid dairy for bloating and assume it is “allergy,” when in fact the issue is simply lactose malabsorption. 

What can be the consequences of food sensitivities? The consequences depend on the type of reaction.

In IgE-mediated allergy 

The risks can be serious and may include recurrent hives, swelling, respiratory symptoms, gastrointestinal symptoms, and in some cases anaphylaxis. 

In food intolerance or non-classic sensitivities 

The consequences are more often chronic and quality-of-life related, such as: 

  • bloating and abdominal discomfort 
  • altered bowel habits 
  • headaches 
  • fatigue 
  • brain fog 
  • skin flares in susceptible individuals 
  • difficulty identifying triggers because reactions may be delayed or cumulative 

These symptoms are real, but they are not always caused by a single antibody-mediated mechanism. In many people, gut health, microbiome imbalance, stress, barrier dysfunction, eating patterns, and overall dietary load may all contribute. This is a clinical inference drawn from how intolerance and non-validated food sensitivity testing are described in current guidance. 

Another consequence is unnecessary long-term food restriction. When people remove many foods without a clear plan, they may create nutritional gaps, anxiety around eating, and an overly limited diet.   • in selected cases, a medically supervised oral food challenge 

For suspected lactose intolerance, evaluation may rely on: 

  • symptom pattern after dairy 
  • dietary trial 
  • sometimes formal testing such as breath testing, depending on the case 

For more chronic and unclear reactions, a structured clinical approach is often more helpful than broad self-testing. This may include: 

  • reviewing symptoms carefully
  • looking for patterns 
  • ruling out true allergy 
  • checking for digestive contributors 
  • using a targeted elimination and reintroduction strategy 

What can be done to treat food sensitivities? Treatment should match the cause. 

  1. For IgE-mediated food allergy 
  • strict avoidance of the confirmed trigger 
  • reading labels carefully 
  • allergy specialist follow-up 
  • emergency plan when needed 
  • epinephrine for patients at risk of severe reactions 
  1. For lactose intolerance 
  • reduce, rather than always completely eliminate, lactose depending on tolerance ● use lactose-free dairy if desired 
  • consider lactase enzyme products 
  • make sure calcium and vitamin D intake remain adequate if dairy is reduced 3. For broader food sensitivities or intolerances 

A more functional and personalized plan may include: 

  • temporary elimination of the most likely trigger foods 
  • careful reintroduction to confirm which foods truly matter 
  • supporting digestive health 
  • assessing whether there is underlying gut irritation, dysbiosis, or barrier dysfunction ● avoiding overly restrictive diets unless they are clearly justified 

The goal should not be to remove as many foods as possible. The goal is to identify what is truly relevant, reduce symptoms, and restore the broadest diet a person can comfortably tolerate. 

A personalized perspective 

Food reactions are common, but they are not all the same. Some are immune-mediated and potentially serious. Others are digestive. Others sit in a gray zone where symptoms are real, but the mechanism is more complex than a simple lab result. 

That is why a personalized approach matters: 

  • identify the reaction type
  • distinguish allergy from intolerance 
  • avoid overdiagnosis 
  • reduce unnecessary food restriction 
  • work on the underlying drivers, not just the symptoms
